On 14 February 1942, Co-Pilot Flying Officer Clive Robert Bernard (268072) and Pilot Flight Lieutenant Elwyn Derek Crisp (267519) were killed when Carmania crashed a few minutes after take-off from Cairns aerodrome for New Guinea. The cause of the crash was unknown and they were the only two on board.
