Aircraftman Carr was fatally injured in a traffic accident at the Meadow Street, Guildford WA, level crossing when a bus carrying RAAF personnel proceeding from RAAF Station Pearce to No. 5 Embarkation Deport and a train collided on 22 December 1941. He died on 24 December 1941.   Aircraftman Class 1 William Charles Lee (17591), discharged from the RAAF on 16 November 1945, was injured.
