Leading Aircraftman Cook, a Driver Motor Transport, died as a result of injuries received in the early hours of the morning of 9 May 1947 when he apparently fell from an emergency escape rope hanging from the second floor window of his barracks block at Bofu, Japan. Various members of the RAAF had been using the rope as a quick means of exit and entrance to the building.
