On 16 February 1942, Kittyhawk AK691 was one of a formation of 5 aircraft which took off when enemy aircraft were reported in the El Adem area. Six Me109s were seen and chased but no contact was made. Later 3 more ME109s were seen and attacked. At 1630 hours AK691 crashed about 20 miles south of El Adem and Flying Officer Threlkeld was killed.
