On 9 January 1945, Beaufighter A9-213 was tasked to attack the docks at Galela, Halmaheras. The aircraft swung to starboard on takeoff at Morotai and struck a stationary bulldozer. The Pilot was fatally injured and died at 1415 hours while the Navigator suffered bruises and shock. The crew members of A19-213 were:
